If you are a current student and would like to schedule an appointment with
a counselor, please complete the form below. We will respond to your request
via e-mail, so please provide a valid e-mail address. EOPS
& DSPS students should contact those departments for
counseling appointments. You can expect a reply within 1 to 2 business days.
Although you can make a counseling appointment for any reason, appointments
are required for the following processes:
Success Contracts or Re-Admit forms (30 min.) for students who are on probation or who have
been dismissed.
Student Education Plans-SEP (60 min.): Students must: 1: A. have completed
Chabot's English and Math Assessments or B. have already completed
Math and English classes at Chabot or another college or C. plan to
start at beginning of Math and English course sequence or D. have AP Math
and English scores of 3 or higher AND 2. have transcripts on file from any
other colleges attended or be able able to bring unofficial transcripts to
counseling appointment.
Unofficial Evaluations-Transcript Review (30 min.): if transferring from
another college, student must have official transcripts on file or hand-carry
unofficial transcripts to counseling appointment.
